We provide care and support services to ten people, male and female, who are aged 18 years and over. The service is located in a built up area on the outskirts of the town. There are extensive local community facilities within close proximity and excellent public transport links. Living here you will have a short assured housing tenancy contract in place with MHM. You will have your own flat and access to all of the communal areas within the building. You will have a key-worker from the staff team who will work with you to identify the outcomes that you wish to achieve and to plan the care and support that you need to help you to succeed. Our skilled and experienced staff team will help you to develop the confidence and skills you need to successfully manage your tenancy contract and to live independently in the community. The staff team will work with you to gain practical skills such as budgeting, shopping, cooking and cleaning and will also help you to find opportunities for social activities, education, training and occupation. We will support each tenant by: Maximising independence Improving social skills, networks and relationships Improving practical skills (e.g. communication, daily living, travel, budgeting, self care) Improving opportunities for employment, occupation and education Improving health and well-being Increasing aspirations and motivation Increasing confidence, assertiveness and self-esteem Increasing social inclusion and reducing isolation Increasing safety and reducing risks Who is this service for? This service is for individuals living in the Middlesbrough area with mental health needs who require support with housing and everyday living skills. How do I access the service? Referrals to the service need to be made by a Social Worker, we will then work with each person to establish their eligibility for the service. The Social Worker will complete a referral form and a risk assessment.
